Community Transition Services (CTS) are non-recurring set-up expenses designed individually for intellectually disabled individuals who are transitioning from an institution to a private residence where they are directly responsible for their living expenses.
CTS provides support and help to intellectually disabled individuals by providing funding at reliable cost. Reasonable or allowable cost and funding are defined as necessary expenses for an individual to establish his/her living space.
Institutions include ICT/MR, ICF/ORC, nursing facilities, or psychiatric hospitals where the patient has been a resident for at least 90(days).
